How To: Add a Questionnaire in a Patient Chart

Need to add information to a patient's chart quickly? With Questionnaires, you can! Get info from patients and have it logged in Nymbl in no time. Follow the steps below to set up your Patient Questionnaires!

How to Add a Questionnaire in Patient's Chart
  1. To add the questionnaire to a patient’s chart, go to Notes > press Questionnaire.


2. The box below will appear, and you can select the name of the form you want to fill out. 


3. Fill out the form with the correct answers and press  Save. 


4. Once you created the form, it will generate a note like this. The answer with be underlined.
  5. You can press  Save Draft  if you need to go back and edit it or  Save & Publish  if the note is completed.
